2014 Fall Soccer Registration

Boys & Girls 4-14 years old (Click here for age chart)
(*Child must have turned 4 years old prior to August 1, 2014 – no exceptions*)
Registration Fee: $40/first player; $20 for each additional player in the household
**Additional $25/child for late registration after May 31, 2014 – NO EXCEPTIONS**
Please bring your child’s medical insurance information as well as a copy of his/her birth certificate. If you provided a copy of your child’s birth certificate for the Fall 2013 or Spring 2014 season, we have it on file and do not need another copy.
All new players on U9, U11 and U13 teams MUST be present @ registration to be fitted for a uniform – cost for the uniform is $23.00 – includes jersey, shorts & socks. If you are a returning player who purchased a uniform in the last two Fall seasons, no new uniform is required. Replacement jerseys, shorts and/or socks are able to be ordered. Pricing for replacement uniform pieces will be available at registration.
All U9, U11, U13 and U15 players MUST also provide a picture AT REGISTRATION (no bigger than wallet size) for required County Player Passes.
Registration fee and uniform fees are required at registration along with parent or guardian’s signature. Cash or checks made payable to “TVYSC” will be accepted for registration.
